message threading bugs

I've just done a major tidy up of message threading bug reports and
what's left is roughly 50 bugs [1]. Many of these are old, longstanding,
annoying. :) I hope some people can take this to the next level and
tidy the list up further, and fix one or two.

[1] contains some testcases, some unconfirmed, some outliers, etc. but
on the whole I suspect there's fewer than 10 different causes behind the
scenes. (excluding mostly ENH requests of meta bug
https://bugzilla.mozilla.org/show_bug.cgi?id=236849 and virtual/unified
folder issues which are troublesome but not of so much interest in the
context of this posting)

Some notable reports:
Bug 638773 - Fake headers (for messages I just sent) are improperly threaded
Bug 719748 - Message view and thread pane easily get out of sync
Bug 812745 - Incorrect threading in the message pane after a move

An example outlier is Bug 876279 - Threaded mail view rendering
incorrect when expanding threads, until mouseover (linux). Driver?
